Dr. Carolina Medina Martín demonstrates the use of a custom double-flange system to secure the IOL in a case of post-traumatic aphakia. This approach aims to give more stability than the single-flange technique the surgical team previously used. The first haptic is passed through both orifices of the double flange, and is secured by melting the ends. The scleral implantation is carried out at 2 mm from the limbus with a 27-gauge needle. As both ends of the flange have to go through the needle, the implantation is performed outside the eye. The second haptic is then inserted with the same method.
