Dr. Ashish Sapkal demonstrates a simple but effective technique to removal cortex and epinucleus in posterior polar cataract using the hybrid irrigation and aspiration method. With this technique, the cortex aspiration is stopped short of central plaque to avoid premature occurrence of posterior capsule rent during irrigation and aspiration. This allows the surgeon to complete the complex task of cortex and aspiration, creating a free floating epinucleus flap that is easy to aspirate in the event of a posterior capsule rent following the posterior plaque separation.
