    During phacoemulsification in an otherwise uneventful cataract surgery, Dr. Ahmed H. Assaf detects a large piece of the nucleus in the anterior vitreous and chooses to sacrifice the posterior capsule, making an iatrogenic opening to retrieve the fragment back into the anterior chamber.
