Iatrogenic Giant Retinal Tear And Retinal Re-Detachment Secondary to Continuous Intense Laser Application During Primary Vitrectomy

A 54-year-old male presented with recurrent retinal detachment following vitrectomy with C3F8 gas. Extensive peripheral scarring and atrophy was appreciated on scleral depression, a result of confluent and intense laser that had been placed at the previous surgery. The retina at the posterior edge of the scar was necrotic and lifted, creating an iatrogenic giant retinal tear. We present this video as a reminder of the potential risks of high intensity confluent laser, and recommend caution if ever performing laser on the continuous setting.
