    Dr. Ahmed Abdelrahman presents a case of 5-year-old boy with Marfan syndrome and massive lens subluxation, where the usual steps were reversed so that the IOL was implanted first and secured to the sclera and then the soft lens was removed. The main advantage of this technique is that the IOL is implanted in a well-formed globe; the IOL is supported by the patient's lens during implantation and fewer peripheral vitreous manipulations are needed.
