    In this surgical video, Drs. Mandelcorn and Felfeli demonstrate an approach to manage a challenging post-trauma case of a dislocated 3-piece Ophtec IOL in the vitreous with an unexpected iris prosthesis (Human Optics, Germany) dislocation. With clever maneuvering, the IOL is explanted, and a secondary lens is inserted and fixated to the sclera behind the iris prosthesis. The iris prosthesis is subsequently refixated to the sclera with a double flanged 5-0 prolene suture. The case is concluded with a well-positioned 3-piece scleral-fixated IOL with a fixated iris prosthesis.
