    Large diameter anterior lamellar keratoplasty can be combined with an ipsilateral posterior lamellar autograft for corneal reconstruction of extreme pellucid marginal degeneration. This novel procedure provides a substantial refractive benefit while avoiding unnecessary introduction of new endothelium, thereby obviating the risk of endothelial rejection and resultant graft failure.
