This video charts the unusual case of an infant who presented with a double crystalline lens. Management involved implanting a specially designed customized acrylic foldable IOL. The initial steps of the surgery followed those of any cataract surgery but later involved the creation of two anterior continuous cuvilinear capsulorrhexes. The surgery was followed by amblyopia therapy.
