This video shows the surgical management of a sub-retinal and sub-ILM (internal limiting membrane) hemorrhage secondary to retinal artery macroaneurysm. The patient had a history of hypertension, along with the macroaneurysm in the right eye that was being treated with intravitreal bevacizumab. She was referred after a sudden decrease in vision. The patient also had a history of poor vision in the left eye due to a hemorrhage from retinal artery macroaneurysm 4 years ago. Vitrectomy, ILM peeling, and subretinal injection of tissue plasminogen activator were performed. Post-surgery, the patient's vision improved from 20/400 to 20/150.
