    Severe Zonular Dialysis and Iridodialysis Repair

    Allon Barsam, FRCOphth demonstrates exquisite cataract surgery in a patient with extensive zonular dialysis and iridodialysis resulting from blunt trauma. After insertion of a capsular tension segment, he proceeds with phacoemulsification under low flow settings with a “stock-and-chop” technique to limit further damage to zonula structures. He inserts a capsular tension ring and a single-piece IOL. Sutures are used to reapproximate the iris root to the scleral wall. At 1 month postop, the patient has a round pupil, minimal iris transillumination and a centralized IOL.
