Mature Cataract and Diabetic Tractional RD Repair

Dr. Rohit Adyanthaya demonstrates how he manages a mature cataract with posterior synechiae in a 41-year-old who had previously underwent diabetic tractional retinal detachment repair. Ultrasound showed silicone oil in the vitreous, but it was difficult to ascertain if the retina was attached. Following phacoemulsification, he inserts a capsular tension ring prior to implanting a 3-piece IOL in the bag. During vitrectomy, Dr. Adyanthaya notes a detached macular along with thick epiretinal membranes. He peels the membranes and performs a temporal relaxing retinotomy to free the stretched macula from the posterior pole. Air-fluid exchange results in a nicely flattened retina, and oil is injected to end the case. Postoperatively, the patient improved from light perception to 20/400.
