This modified technique of pre-Descemetic DALK is introduced for the primary treatment of acute hydrops at presentation, giving early and effective visual (UDVA and CDVA), anatomic, biomechanical & topographic rehabilitation, less topo and refractive astigmatism, less dependence on contact lens and a better contact lens fit when required. Most importantly, it avoids healing by scarring unlike in conventional management, thereby preventing the need for penetrating keratoplasty.
