Despite increasing popularity of endoscopic lacrimal and aesthetic procedures, endoscopic orbital decompression has not been widely practiced in the ophthalmic community, potentially related to the lack of specialized equipment and training of endonasal anatomy. This video summarizes an original, ‘3G approach’ from an ophthalmologist’s 10-year experience, which describes a strategy in personalizing endoscopic decompression that is graded towards extent and indications of decompression, guarded against complications, and guided by endoscopic navigation and landmarks. While an endoscopic system allows unmatched illumination, magnification and recording, the “pressure-less” transethmoidal approach is of paramount importance especially to a compromised, compressed orbital apex.
