- Steep incisions resulting in injury at the capsular rim
- A leaky phaco sleeve and twisted infusion tube causing chamber instability
- Hydrorupture of the posterior capsule
- Conjunctival overhang and ballooning obscuring the surgical field
- Accidental emulsification of the iris in a case of small pupil
- Ruptured zonules during irrigation and aspiration
- Projectile IOL injection leading to floppy pupil
