During cataract surgery on a 50-year-old man with diabetes, the surgeon makes an unexpected finding: multiple opalescent cysts on the posterior capsule. They appear to be part of the posterior capsule and will not detach. Surgery proceeds with IOL implantation. Postoperatively, the cysts are unnoticeable.
