Dr. Lisa Arbisser offers an approach for dealing with breaks in the posterior capsule during cataract surgery. The surgeon faces a tear that was too large to convert to a posterior capsulorhexis. She stabilizes situation with a dispersive viscoelastic to maintain the anterior chamber. She then places a three-piece lens in the ciliary sulcus and performs an optic capture to secure the haptics.
