This video demonstrates a repair of retinal detachment with vitrectomy 1 week after cataract surgery complicated by suspected retrobulbar block perforation. Upon entry, a dense vitreous hemorrhage and a large inferior subretinal hemorrhage was noted, along with inferior and temporal retinal detachment. During core vitrectomy, the subretinal hemorrhage was aspirated via the breaks. Triamcinolone was injected, and residual preretinal vitreous gel was removed. A 360-degree depressed shave, air-fluid exchange, the subretinal fluid was drained and endolaser was applied to end the case.
