Drs. Omesh Gupta and Daniel Su demonstrate scleral fixation of a MX60 lens (Bausch + Lomb) using Gore-Tex suture. A toric marker is first used to mark the 3 and 9 o’clock positions, followed by a relaxing conjunctival incision and 3 25-gauge sclerotomies. Next, the Gore-Tex suture is threaded through the eyelet and over the haptic. After folding the lens, the leading haptic is tucked into an IOL “taco” and inserted into the eye while maintaining constant tension on the suture. They use a 3-loop throw to temporarily stabilize the IOL while positioning the trailing haptic and externalizing the other sutures. The sutures are tied using the 3-1-1 technique, and the tension is adjusted to properly center the IOL. Finally, the knots are trimmed and rotated into the sclerotomies.
