Dr. Gowtham Jonna manages a case of an unexpected intraocular foreign body. During instillation of brilliant blue dye, the entire silicone sleeve of the soft-tip cannula is dislodged into the vitreous cavity. This occurred despite priming the cannula outside the eye. The silicone sleeve is eventually removed through an enlarged sclerotomy with the aid of Maxgrip forceps. The internal limiting membrane is peeled and air-fluid exchange is performed to end the case.
