Dr. Suven Bhattacharjee warns that unexpected complications may occur during phacoemulsification. In this case, he failed to notice shallowing of the anterior chamber as he emulsified the last fragment, resulting in the iris prolapsing and the eye becoming hard. Although he managed to reposit the iris, the eye remained hard and he was forced to return to the procedure at a later time to safely complete the treatment.
