This video presents the case of a patient with advanced juvenile rheumatoid arthritis. The patient's left eye was noted to have only light perception, an anterior uveitis associated inflammatory membrane, shallow chamber, peripheral anterior synechiae, small pupil, and a white cataract. The surgical plan involved phacoemulsification with IOL. After anterior chamber reformation and membranectomy, the pupil is opened using Grieshaber hooks and a 180-degree zonular disinsertion is observed. The authors proceed with an intracapsular cataract extraction, sectorial iridectomy, and anterior vitrectomy.
