Dr. Tamhankar argues that the common consensus is that patients needed neuro-imaging and recommends early MRI in all patients with ocular motor cranial nerve palsy. On the flip side, Dr. Quiros argues that even though all III and IV nerve palsies should be scanned, there are drawbacks to scanning including gadolinium toxity andfalse-negative rates as high as 30%.
