This video highlights the detailed preoperative evaluation of cases of microbial keratitis with regard to the extent and depth of infiltrate, presence of perforation and nonhealing keratitis which necessitate therapeutic keratoplasty. The intraoperative course can present a variety of unexpected and complex situations that require modification of the surgical technique and a more customized approach. Different challenging scenarios in therapeutic keratoplasty and how to approach them intraoperatively to achieve good anatomical success are presented here
