In this presentation from AAO 2016, Heather E. Moss, MD, PhD, presents a framework for optic neuritis. She divides it into typical (multiple sclerosis), atypical (neuromyelitis optica type), and other optic neuritis, which can either be isolated or in a patient with no disease. For diagnostic tests, she advises for the typical MS patient clinical diagnosis MRI for MS risk; for NMO patients, clinical diagnosis anti-AQP-4 LgG; and for other ON patients, serum tests, neuroimaging, and lumbar puncture. For all 3 categories, the clinical features remain the same: severe bilateral vision loss, progressive vision loss with unclear diagnosis.
