The trend toward selectively replacing pathologic corneal tissue has led to an increase in lamellar corneal surgery.

Endothelial keratoplasty (EK) is now the procedure of choice for treating endothelial cell dysfunction. Advantages include preservation of structural integrity, faster visual recovery, and more predictable refractive outcomes.

Deep anterior lamellar keratoplasty (DALK) preserves the host corneal endothelium, eliminating the risk of endothelial rejection and providing additional structural support in the case of trauma. However, DALK does not reduce postoperative astigmatism or prevent stromal rejection.
