Small tears in the anterior lens capsule are sealed by nearby lenticular epithelial cells. In circumstances that make the lenticular epithelium anoxic or hypoxic, such as posterior synechiae or markedly elevated intraocular pressure, a metaplastic response may occur, producing fibrous plaques intermixed with basement membrane.
