Macular Holes in High Myopia
A distinct variant of RRD is caused by macular holes, almost always in the setting of a posterior staphyloma in highly myopic eyes (Fig 16-20). Vitreous cavity fluid enters the subretinal space through the macular hole and initiates the detachment. These macular holes with retinal detachment have a far lower success rate for surgical repair than do either macular holes or typical RRDs.
