Dr. Ahmed Abdelrahman demonstrates a combined nasal goniotomy–temporal trabeculotomy (NGTT) procedure in a patient with primary congenital glaucoma. A standard nasal goniotomy is first performed, using a microvitreoretinal or 27-gauge bent needle. After completing the goniotomy, Dr. Abdelrahman makes a lower temporal scleral flap triangle and then a second flap is dissected down to the Schlemm canal. Following that, a trabeculotomy is performed on the temporal side. A few days after surgery, the superior conjunctiva is completely spared.
