Pars Plana Vitrectomy for a Plugged Glaucoma Tube

    Glaucoma, Surgical Management

Dr. Marta Stevanovic and colleagues describe the case of a 36-year-old woman with a history of juvenile idiopathic arthritis, advanced secondary glaucoma in both eyes, a 13-year-old Ahmed valve placement in the right eye, and a Soemmering ring cataract in the right eye who presented to the emergency room with pain, pressure, and intermittent blurry vision. The right eye had a visual acuity of 20/25 and an IOP of 38 mm Hg. Her examination was notable for vitreous prolapse through the pupil with vitreous material plugging her Ahmed tube. A pars plana vitrectomy was performed to remove the vitreous that was plugging the tube and to relieve obstruction of the peripheral iridotomy by the Soemmering ring cataract. The IOP normalized to 12 mm Hg within 7 days of the procedure, indicating a successful surgery.
