Tips for Phakic DMEK on a Younger Patient

Drs. Arthur Okonkwo and Ahmed Bardan perform phakic Descemet membrane endothelial keratoplasty (DMEK) on a patient in her 30s with early onset Fuchs endothelial dystrophy. The procedure begins with a descemetorhexis that measures larger than the graft. Use of Healon increases space for instrument manipulation. The viscoelastic is later removed to improve graft adherence. Following insertion, unfolding, and centering of the graft, the procedure concludes with an air tamponade to prevent premature cataract formation.
