Is it possible to get posterior capsular opacification without having cataract surgery?

Answer:
No, this only develops after cataract surgery. One can develop posterior subcapsular cataract that clouds the center part of the lens in front of the posterior capsule of the lens. This is more common in young people with cataract, or cataract resulting from chronic steroid use, or from trauma to the eye.
