Would a cataract surgeon be able to identify a rotated IOL during an examination?
Assuming you are talking about a toric lens (astigmatism-correcting IOL) being possibly rotated, the answer is yes. As long as the pupil is dilated wide enough to get a good look, this should not be a problem.
