    Idiopathic Vasculitis

    A healthy 30-year-old woman presented with sudden onset of blurred central vi­sion (20/200 OD and 20/100 OS). Oph­thalmic examination was significant for bilateral cystoid macular edema confirmed on OCT. Intravenous fluorescein angiography showed extensive perivenular dye leakage starting in the arteriovenous phase (Figs. 1, 2). Petaloid peri­foveal leakage and leakage from the optic disc appeared in the late phase (Figs. 3, 4).

    A thorough systemic evaluation, including blood work and chest x-ray, was unremarkable. Her vision returned to 20/20 after a 10-day course of oral steroids and has remained at that level for more than a year. A follow-up angio­gram 2 months after presentation showed reso­lution of vasculitis and macular leakage.
