    Transposition of the split LR muscle for correction of eye misalignment associated with third nerve palsy. a. A superior oblique muscle tenotomy and inferior oblique muscle myectomy are performed. b. The LR muscle is split into superior and inferior halves posteriorly for ≥ 15 mm, secured with a standard muscle suture, and disinserted from the globe. c. The superior and inferior halves of the LR muscle are transposed to the nasal side and are reattached to the sclera with an adjustable technique: the insertion can be up to 2.5 mm anterior to the MR muscle insertion or recessed from this point appropriately.
