Your evaluation of the optic disc should be recorded. This record can be a drawing of the appearance of the optic disc or a verbal description. In patients at high risk for glaucoma, baseline optic nerve photographs or images may be desirable and can be obtained in conjunction with a comprehensive ophthalmologic examination. These images are useful for future comparison to document change.
