Drs. Davinder Grover and Arsham Sheybani discuss changes over the years in their technique for implanting the XEN Gel Stent, a MIGS (minimally invasive glaucoma surgery) stent, to reduce IOP in patients with glaucoma. A newer method is to perform a posterior sweep of Tenon capsule, or POST, in which a microshunt spatula with sharp sides and a blunted tip is used to tunnel away from the stent. It is similar to a primary needling technique, but the risk of perforating the conjunctiva is lower with use of the microshunt spatula. Using this method, Dr. Grover has noticed improved bleb morphology and more predictable microcysts in his patients.
