    Question: I performed a new-patient exam, level 99204, along with a probe and irrigation, CPT code 68810, due to an eyelid laceration involving the tear duct. The following day, I repaired the laceration with an adjacent tissue transfer and a canthoplasty, CPT codes 14060 and 67950. What modifiers do I use for both dates of service?

    Answer: Medicare does not require you to append modifier -25 on a new-patient exam when you perform a minor procedure the same day. Submit the exam along with 68810 and the eye modifier. 

    Since it sounds like you planned this at the time of the original procedure, append modifier -58 to both procedures along with the eye modifier.
