Chart-Note Completion Deadline
Question: Is there a rule stating that the chart note must be completed within a certain number of hours of the exam’s completion?
Answer: Only that it is to be completed in a reasonable timeframe. Payers expect you to generate documentation at the time of service or shortly thereafter.
