Answer: Submit CPT codes 68525 Biopsy of lacrimal sac and 68840 Probing of lacrimal canaliculi, with or without irrigation. Typically you should list CPT code 68525 first, as it has a higher allowable. There are no CCI bundling edits between the two CPT codes.
