Answer: Superficial lesion removal codes fall under the integumentary category; codes from the ocular adnexa include more depth. Consider either 11440 Excision, other benign lesion including margins or 67840 Excision of lesion of eyelid (except chalazion) without closure or with simple direct closure and document the size of the lesion.
As a safety net for payment, the Academy recommends you obtain an Advance Beneficiary Notice from any Medicare Part B patient for any oculofacial procedure that CMS could deem cosmetic. Remember to append modifier -GA to the CPT code.
