    Does Medicare still require preauthorization for blepharoplasty?

    Yes, prior authorization is required for blepharoplasty when performed in a hospital outpatient facility. CMS did remove CPT 67911 from the prior authorization list, see the Academy’s January 2022 announcement: CMS Removes Lid Surgery Code From Hospital Outpatient Prior Authorization List. Other blepharoplasty and ptosis codes are still on the updated April 14, 2022 prior authorization list.
