• Functions of the VF-8R form

    Question: We created our own activities of daily living (ADL) questionnaire for our cataract surgical patients Do we need to use the VF-8R form instead?

    Answer: When the payer does not have a visual acuity requirement, the best supporting documentation to your exam findings is the patient’s complaint. The VF-8R form serves three functions: 

    1. It is a nationally recognized form that is required by some of the Medicare Administrative Contractors (MACs).
    2. It is the preoperative form required for PQRS if you are reporting via the Cataracts Measures Group, and
    3. If your operative report does not detail the indications for surgery, it meets medical necessity requirements should the facility be subject to an audit.
