Functions of the VF-8R form
Question: We created our own activities of daily living (ADL) questionnaire for our cataract surgical patients Do we need to use the VF-8R form instead?
Answer: When the payer does not have a visual acuity requirement, the best supporting documentation to your exam findings is the patient’s complaint. The VF-8R form serves three functions:
- It is a nationally recognized form that is required by some of the Medicare Administrative Contractors (MACs).
- It is the preoperative form required for PQRS if you are reporting via the Cataracts Measures Group, and
- If your operative report does not detail the indications for surgery, it meets medical necessity requirements should the facility be subject to an audit.
