    Question: For coding purposes, what are the guidelines on the questions we must ask for activities of daily living prior to cataract surgery?

    Answer: Not all payers require a formal ADL form, so always verify with each payer. A nationally recognized form is the VF-8R that lists eight questions. If you choose to create your own, these questions can be a guideline. The VF-8R is available on the coding updates and resourses webpage. When required, we recommend using this form because it is nationally recognized. When required, you should also provide a copy to the facility because it demonstrates medical necessity.
