    Is it necessary to do confrontational visual fields when coding any level of an exam? 

    Answer:
    Confrontational visual fields are required for comprehensive Eye visit codes (92004 and 92014).

    With the 2021 and 2023 evaluation and management (E/M) guidelines, documentation requires a medically appropriate history and medically appropriate exam, both determined by the physician. Involve physicians in the training of the technicians to instruct the necessary history and exam elements to be performed.
