  • Reporting Wastage in Box 19 on Claim Form


    Question: Should we report what was injected in the eye only, or if there is reportable wastage, do we include this number in box 19?

    Answer: When using a single dose vial, like Triesence, and you have measurable wastage of 39 mg, be sure to bill a second line with modifier -JW append to J3300. This tells the payer the units discarded. Only include medication name, dosage and/or NDC in box 19 per payer guidelines. Confirm the chart documentation clearly states the amount of medication injected and wasted. For anti-VEGF drugs that do not have measurable wastage (less than 1 unit), document “any residual medication less than one unit was discarded.”
