What diagnosis codes should be reported?
Question: What diagnosis codes should be reported? All that apply to the patient or all that apply to today's visit?
Answer: The primary diagnosis should relate to the chief complaint. Only report those diagnosis codes that pertain to the exam. While you can report up to 12 diagnosis codes, it is certainly not required for payment.
