This presentation reviews the challenge of positive posterior pressure with anterior chamber shallowing after lens removal during cataract surgery. Dr. Deepinder Dhaliwal noted positive pressure and chamber shallowing in a 92-year-old man immediately after cortical cleanup and removal of the irrigation/aspiration handpiece; she suspected aqueous misdirection. Experts and the audience evaluate next steps after the chamber did not deepen with additional ophthalmic viscosurgical device.
