    In this surgical video, Dr. Ahmed Shalash uses a combined technique of needle decompression and double rhexis to manage an intumescent cataract. He believes this technique provides a more predictable outcome and a less challenging surgery. Intumescent cataract surgery can be quite challenging due to the raised intralenticular pressure, which can lead to radial extension of the anterior capsule resulting in the classic Argentinian flag sign. Needle decompression or a mini rhexis may not be sufficient to reduce the pressure inside the capsule.
