Drs. Abbey and Wolfe describe a novel approach to scleral fixation of an IOL through trocar cannulas. They demonstrate it here in a 63-year-old patient with a dislocated 3-piece IOL, initially placed in the sulcus, and a history of scleral buckle and vitrectomy for retinal detachment. After performing a pars plana vitrectomy, they rescue the dislocated 3-piece IOL using transconjunctival fixation through trocar cannulas. This technique has resulted in significant visual improvement and minimal postop complications.
