Effects of Artificial Endothelial Layer in Complex Rejected DSAEK Patients

    AAO 2023 Video Program
    Cornea/External Disease, DMEK

Case 1: A 63-year-old female with a history of uncontrolled advanced glaucoma underwent several filtration surgeries, resulting in endothelial decompensation in her left eye. She underwent a Descemet-stripping automated endothelial keratoplasty (DSAEK) procedure, but her graft failed 10 months after the operation. An artificial endothelial layer (EndoArt) was successfully implanted after two rebubblings were performed. While her visual acuity only improved from hand movement (HM) to counting fingers due to the advanced glaucoma, the central corneal thickness (CCT) decreased from 1658 μm to 684 μm at the 6-week and 1-year follow-ups. Case 2: A 57-year-old female with a history of left eye trauma 15 years ago underwent corneal suture with cataract extraction, followed by DSAEK due to pseudophakic bullous keratopathy. Unfortunately, she experienced graft failure 16 months after the DSAEK procedure. An EndoArt implantation was performed, resulting in improved visual acuity from HM to 6/20, and a decrease in CCT from 795 μm preoperatively to 506 μm at the 3- and 12-month follow-ups.
